Co-Brand With Us
The Department of Healthcare Services is proud to offer Smile, California co-branded materials for county agencies to use to promote oral health. The co-branded items will display your organization’s logo next to the Smile, California, and the Department of Healthcare Services logos.
To submit a co-branding request, fill out the form in the link below. Once the request is received and submitted for State approval, a Smile, California outreach representative will reach out to let you know if you have been approved. Please allow up to 4 weeks for delivery of co-branded materials. Please refer to the “Materials for Medi-Cal Members” tab in the Partner & Providers page to view a list of the available materials and the languages they are available in before submitting a co-branding request form.
